KidSight - Referral Follow-up Information
If your child was referred by the KidSight Outreach or KidSightPlus vision screening programs complete the form below to let us know when they have been examined by an eye care specialist or an appointment has been scheduled. If you have questions, please add them int he comments section.
If you need assistance with the cost of the exam or glasses, please contact our offices. We will put you in contact with your local club for assistance with this cost.
